Barnes and Noble, Inc. is an existing plan with CVS/caremark. Effective August 1, 2015, Barnes and Noble, Inc. and Barnes and Noble Education, Inc. are splitting into two separate companies.

The "Louisiana Addendum to Caremark Provider Agreement", beginning on page 98 of the Caremark Provider Manual, is amended to add the following new section, after section 5, as follows:

Effective immediately, the “Non-Statin Cholesterol PDC” adherence measure is no longer an adherence measure for the CVS/caremark Adherence Improvement Program.
