Catamaran, an Optum Rx Company, is providing notice of COB Segment processing changes for Healthcare Alliance Medical Plan. These changes are being made to align with the HFS-IL Medicaid Third Party Liability Billing instructions. As a result it is important to note that Other Coverage Code (OCC) 8 COB claims will no longer be accepted and pharmacies must submit the applicable OCC 2, 3, or 4 after sending to the Primary Payer. Specific details as to acceptable data in the COB segment for members that have been identified as having Alternate Insurance are included on the following page.

Dear Healthcare Professional: On December 1, 2015, the FDA announced that Insulet initiated a voluntary Field Safety Notification of some lots of the OmniPod insulin management system due to a slight increase in the reported cases in which the OmniPod’s needle mechanism failed to deploy or there was a delay in the deployment of the needle mechanism. The incidence of this issue in the affected lots is approximately 1%-2%. Per Insulet, the higher rate of failure in the OmniPod insulin management systems could result in the interruption of insulin delivery which can cause hyperglycemia, and if left untreated, can result in diabetic ketoacidosis. To date, the affected lots have resulted in 66 medical device reports of which 3 required medical intervention.
