the member’s Medicare D coverage first, then submit the Medicare D copay as secondary to
the First Farmers’ Wrap Plan. This will process to OptumRx under: BIN: 610127, PCN 04000013,
GROUP: 01961925, OTHER COVERAGE CODE 8. The member is responsible for the copay
returned from the second pharmacy submission. Pharmacies needing assistance may call our
Customer Service at 1-877-559-2955. Please request a copy of the member’s ID Card. Please do
not refer members back to their Plan Sponsor.

Impax Laboratories, Inc. issued a voluntary nationwide retail level recall on August 19, 2016 for one lot of Lamotrigine Orally Disintegrating Tablet (ODT) 200 mg, NDC 0115-1529-08, Lot # 502240. The affected lot was distributed between June 13, 2016 and August 10, 2016 to wholesale distributors and retail pharmacies nationwide.
