Aetna: 2017 Medicare Part D Plan‐to‐Network Crosswalk

Effective January 1st 2017

This document outlines which Aetna retail pharmacy network is associated with each 2017 Aetna/Coventry MAPD and PDP plan.

Aetna will use the same Medicare retail pharmacy networks in 2017 as in 2016.

  • P1: A broad network with both preferred and standard cost‐sharing components.
  • S2: A broad network without a preferred cost‐sharing component.

US Script: Sunshine Health - DISCONTINUATION OF SPECIFIC DIABETIC BLOOD GLUCOSE TESTING PRODUCTS

Effective June 1st 2016

Starting June 1, 2016 Trividia Health will no longer supply NDCs for diabetic blood glucose TRUEtest test strips.

The TRUEresult® and TRUE2go®blood glucose systems use TRUEtest test strips, which are GDH-PQQ enzyme-based test strips. For a very small percentage of the population with diabetes, the GDH-PQQ enzyme can interfere with certain drug products or therapies that contain non-glucose sugars. The manufacturer is phasing out the TRUEresult and TRUE2go blood glucose system and will work with you to transition your patients to the TRUE METRIX blood glucose system.

PHARMACIST: Dispense a TRUE METRIX® or TRUE METRIX AIR Meter at no cost or copay to the patient. Process the claim using the processing information listed below. Please use a prescription on file or call the patient’s physician to obtain a new prescription.

Mirixa: Humana Star Ratings Program Re-launched

Humana Star Ratings Program Re-launched!

Up to $40 for Achieving Adherence Goals

 

Mirixa will soon re-launch an updated Humana Star Ratings Program. The goal of this program is to improve patients' adherence to target medications, by ensuring that patients have a sufficient quantity of each medication to achieve a Proportion of Days Covered ("PDC") greater than 80%, and by counseling patients on the importance of adherence.

 

Cases will be presented with "General Adherence Counseling Alerts," and/or "Conversion Counseling Alerts." The Conversion Counseling Alerts encourage pharmacists to fill a patient's prescription with a 90-day supply of medication rather than a 30-day supply, as a means of improving the patient's adherence. 

 

Mirixa will pay the pharmacy up to $40.00 per alert, payable in one or two installments per the terms below. To receive payment, pharmacists are required to resolve each alert by:
